Welcome to Churchwood Surgery
The Practice will optimize the quality of care, control of symptoms and the prevention of illness
We are a long established practice previously based at High Street Surgery. We moved to Pontypool Medical Centre, our new purpose built surgery in 2002. Since this move we have gradually expanded our range of services, many of which are outlined on this site. We work closely with other health care professionals to run an efficient primary health care team. We have facilities for the disabled and have a car park at the front of the building.
We also have a branch surgery in Goytre where we are able to dispense prescriptions immediately to those patients who live in the area and are registered with this surgery.
With patients' needs at the heart of everything we do, our website has been designed to make it easy for you to gain instant access to the information you need. As well as specific practice details such as opening hours and how to register, you’ll find a wealth of useful pages covering a wide range of health issues along with links to other relevant medical organisations.
Get Well, Keep Well
Of course we’re not just here for when you are unwell. Our team of healthcare professionals and back-up staff offer a number of clinics and services to promote good health and wellbeing whatever your medical condition.
We hope you enjoy having a look around the site and familiarising yourself with some of the online features such as ordering a repeat prescription. Whatever your thoughts, be sure to let us know via our feedback function. Comments and suggestions are always a great way of helping us continue to enhance the way we look after you.
My Health Online is a new online service brought to you from NHS Wales. My Health Online offers patients the convenience to book appointments using the internet and order repeat prescriptions online or change your contact details. If you wish to register to use My Health Online please contact the surgery. To protect patient confidentiality you must provide documents so we can make sure you are who you say you are. When you next visit the surgery please complete the MHOL registration form and provide identification, preferably photo ID (e.g. driving licence or passport).
